I clicked the link, typed in a characters name. Then in each of the following I typed:
Name - Test
Number of Dice - 4
Sides of Dice - 20
Modifier - 4
Note - Test Roll
Then I got a page that looked like this:
Test roll
Done!
ID = 200501
Username = Test
Note = Test roll
Date/Time = 8/24/2008 7:45:34 AM
Dice = 4
Sides = 20
Mod = 4
Rolls = +18 +10 +11 +2 +4
Total = 45
You can link to this record with the following URL:

Add new roll to database.
Search for results by username.
Then simply click on the addy:

At least it works fine for me.
