From playing at XP, this has been the thing that has stuck out. D&D has always been, lets call it abstract, and heroic...but at some point it can feel a little too silly. Especially given the pounding a PC can take on a long 4E day.

Mouseferatu/XRPs Advanced Player Guide also has some interesting wound rules.

Great addition to the game. One of the e-dragons, and the FRPG had more rituals, and I think these are in the compendium. If you have access.
