In most games if a PC does a lot of damage and kills an enemy I might describe gruesome endings such as decapitation, limb removal, guts spilling onto the ground, etc., etc. but it's not at all important that this is represented in the mechanics of the game. I can't say that there would often be a situation where it was important to a story what traumatic injuries lead to character death, and in my experience permanent maiming tends to be an even more unsatisfactory end to a character than death.     How important is it for you to have the possibility of decapitation for any character unlucky enough to receive damage? What should the chance of a character being maimed be?     Another great resource I use in my Primeval Thule campaigns is the World of Xoth stuff by Morten Braten. It's all written for Pathfinder so it's pretty easy to convert. The website has a couple of freebies but the main PDFs are really reasonably priced. http://xoth.net/publishing/

Some are campaigns where the DM only wants to run for a finite period, and ends the game when the DM is no longer willing to continue.     For my part, if I were to redesign 5e, the only class that I would not change at all would be Rogue. I think it has, by far, the best overall class design of the entire edition.     and yet again, we see more info on your game which helps show how little strength matters there due to campaign specific parameters. My first level halfling has like 90lbs of get off the bat and thats before moving to decent (heavier) armor or weapons. After so many ways to devalue strength being chosen by your group, its not surprising it seems underbalanced.

    Gold weight is rarely tracked in any game I've seen. You can always exchange for gems or get a pack mule. I have a rogue character I haven't run yet and I just added a bunch of typical gear. He even has an extra set of armor because I forgot to remove a set. He's not encumbered yet. I don't remember ever seeing an issue unless the PC wears medium armor.

    I've noticed that just a few giants can do obscene amount of damage even to PCs with a decent AC. My son attacked 3 fire giants with his dragonborn Bbn-19 expecting an easy fight, and got a major ass-whupping :D

    The biggest thing I've noticed at the highest levels is that it is actually easy to build an encounter that kills PC's. Lots of those high CR monsters can do huge amounts of damage so one lucky round can drop a character. When my HotDQ/RoT group (six 17th level PC's) fought Tiamat, they only succeeded because they had a life cleric who Mass Heal to basically reset the entire party all at once...

    I could use this building as a Haunted Barracks, and fill it with skeletons and zombies. Like hundreds of skeletons and zombies. And include some treasure in one or two rooms per floor. And I'm real tempted to plot out a series of "patrol paths" like those that video game guards follow in stealth games.

    Luke didn't really have to become more powerful. But I can't see how a Luke that turned Darth Vader back from the Dark Side of the force, a Luke that risked life and limb (literally) to try to save his friends on Bspin or from Jabba the Hut, would turn his back on his best friends, his family in a time of need.     I think it is fair to say 4E was very balanced (way more than 3E was in my opinion). I don't that is bad. It is just where things were at the time.
