Just 1 although I require a 2 squares verticle to be over a medium sized creature.

Ah, I thought you meant, move one diagonally level *then* move one diagonally up.

It would be a single square of movement to move 1 square North, West and Up, all in one swoop.

However, it depends on the way you move. If you need to climb to go up, it may have movement penalties associated with it.
