Depends on what setting and what the DM is using, but typically:

Player's Handbook
Possibly another book or two depending on the setting or character type (Spell Compendium, Player's Guide to the Realms, Oriental Adventures, ect.)
Character Sheet (typically left in the PHB from last week, unless I had to print another one out)
Pencil
Dicebag
A Miniature (if minis are used and the DM isn't furnishing them, which I do when DMing and many DMs I know do the same)
Maybe something to eat/drink or money to order food depending on what the typical eating arrangements are for the game.

Scribbles from previous session
Dice
Pencils
Pens
Eraser
Snacks (usually Doritos, fresh fruit, salsa, or baked goods)
(sometimes) laptop with the standalone version of d20srd.org

When I'm not DMing, and not at home (not often, but I recently was in a game like that), PC sheet, pencil, dice, snacks, relevant books. The awesome is a given.

We usually play at my place, so I tend to "bring" the venue, including music and (hopefully not too un)comfortable seating.

I also stock a selection of beer, whiskey, rye, and single-malt scotch.

Hm, let's see...

Food/drink offering appropriate for the group and venue
Dice appropriate for the game.
Writing implement.
Rulebooks that I expect I need for the session.
Some paper and a writing surface (sometimes a clipboard, sometimes a folder or rulebook)
Character sheet - for some games this is one piece of paper, for other, long standing campaigns in systems that call for it, it turns into a folder or binder.

That's usually about it.

My new group goes old school for this one:
All players have a character in document protectors with a mechanical grease pencil that the DM keeps. Those that have one bring their phb and their dice. We usually have dinner for the group (which donatioins are accepted, but never asked for), but players usually bring moeny in case we order the Dominos 5 for $5 deal.

I mostly DM. That said, even if I'm a player, things don't really change much.

I bring all my dice. Never know when one set will run cold. (about 100 sets give or take).
My minis.
All relevant DM material I've printout out that week (ie dungeon maps, DDI stuff, etc).
Campaign/World Notes kept in my notebook.
Pre-generated lists of names for Inns/Shops/etc (I'm terribad at coming up with them on the spot)
Pre-generated list of regular names (see above)
I bring my entire library of whatever game we're playing, just on the off-chance I might need something.
A couple cans of Pop and some cash for food.
A handful of mechanical pencils.

All in all, ends up around 1 duffle bag of books, 1 book bag full of books/misc stuff, 1 box full of minis, and 1 box of dice, subdivided into three color coded mini-boxes.

If I bought a laptop, I'm guessing I could cut down on my clutter a ton.
