I have Knight in my custom title, does that count?
It comes from a character I played last campaign. She was a knight, and the first encounter involved us being woken up in our barracks, surprised by goblin raiders. I declared that my character charged out in her nightie, and for the rest of the campaign she remained the "Nightie Knight", even though I think that nightie only came out one more time in the rest of the year-long campaign.

Strangely I think I'd be okay with having a PC die by a bear exploding out of his head. --Awayfarer, after having his PC put a furry ball from a Bag of Tricks into his mouth.

I didn't specify in the game. It was just "a nightie".

I had a female Cleric/Ranger that was once captured and held dressed in her night clothes. When she got free I went ballistic. I was hot with the dice and nearly took out all the guards and even the fleeing ones before everyone else was free.
A lot of people respected her more after that. "Don't get her mad- you wouldn't like her when she's mad"
