font and logo?

Kiznit - what font did you use and where dif you find the logo? I wanted to do one of these cribs for initiative and surprise (I'm DM-ing my first game)

This is really well done!

Minor comment:
On the modifiers table, "Target has concealment" and "No line of sight" appear identically under Melee Attacks and Ranged Attacks. You might want to consolidate them under "All Attack Types." Also, the most common situations that grant combat advantage should be on the chart, maybe listing "CA" instead of a bonus or penalty.
