I can't take full credit for this as I have modified a template I stole many years ago and have been playing around with ever since. Its a work in progress, so I am looking for constructive feedback.

Feedback?

I think this sheet is rather useful, I'm going to use it to record some NPC's I'm working on for a new campaign.

I made some changes to swap the positions of dex and con stats to more closely match the character sheet.

Since the deities are mentioned by name in a few of the feats, there should be a place to record the deity.

Also, I had to reduce the default font sizes on the right-side fields on page 2 from 10 pt to 8 pt font so the fonts are not cutoff by the cell border underlines.

BenzoR, I made most of the changes you mentioned to the sheet except for the Diety, and updated the original post. I'll have to find a spot for that in the next revision. It was in my original, but came out for space.
