Apologies if there's already one of these floating around, but I think it is important to memorialize those lives that have been cut short by the darkness between the points of light. With that, my first 4E PC death (and also my first character):
Domnorix - Wladrin Warlord. He died while leading from the front, shield up, sword in hand and a half dozen goblin arrows in his chest. If his brother in arms (and perhaps more? we shall never know) had not completely flubbed his heal check, he might have lived to fight another day.
