Sneak Attacks Range

There's a feat in PHBII called Deadeye shot. To use it, you ready an action to fire a missile at a target that has just been struck by an ally. The target looses his dex bonus against your readied shot. There is no range limit on applying sneak attack from this feat.

This is misleading. You still need to be within sneak attack range in order to sneak attack. And Deadeye Shot doesn't give you a sneak attack when someone is hit in melee. It makes your readied attack catch the foe flatfooted, or makes him lose dex to AC (which isn't always the same thing, in regards to Uncanny Dodge, etc...), I forget. Which means that if you're within range, it should be a sneak attack.

Another option if you absolutely refuse to just use frigging Sniper's Eye spell is to beg your DM to import a variant rogue archetype (basically like an alternate class feature) from Pathfiner, the Sniper. Trading Trapsense and Trapfinding for the benefits it grants:

Accuracy (Ex)

At 3rd level, a sniper increases the range at which she can apply her sneak attack damage by 10 feet. This range increases by 10 feet for every 3 levels after 3rd.

This ability replaces trap sense.

Well yes, they should BOTH get flanking bonus. Otherwise they're not flanking. Sounds like the rules are just presented unclearly. Yay, RC!
Someone did not proof read the diagram very well. It has a +2 under the name of anyone getting the flanking bonus, then a text block in a corner explaining why they got the bonus. The Orc should have a +2 under his name.

Because you are not flanking as defined by the game rules.
We allow it because it is in the spirit of OD&D rules. Someone engaged in a melee is focusing their attention on the fight and can loose track of someone circling around to make a surprise attack from an unexpected direction.
