It doesn't have to be a horse. But I think it comes from another plane, not the real world location you're in. But the GM ultimately decides, regardless of the specifics of it.

I have plans for that steed, either way, so I hope the Paladin is a fan of either giant lizards, oversized toads, big-ass snakes, or dinosaurs are cool with you.

I'm thinking mounted combat, but not a Paladin. Fighter. I don't want to deal with spells in this case. I am happy with weird mounts.

Lieutenant Markus Beauregard. Called "Lefty" by his peers (and his subordinates behind his back, though he doesn't mind) both as short for lieutenant and because he's left-handed.

He loved that horse. Served him well in previous campaigns. Damned ship that can't stay afloat!
