Part of the same power does not mean same attack. It's a seperate attack from the initial, therefore not including the fighter subjects it to combat challenge.
Swordmages almost never have a poor fort, as str and con are both secondary stats for them. Swordmages generally seem to have the most well rounded defenses in my experience.

They have lower attack rolls because most of their attacks are implement attacks that don't target AC, so they're not getting their proficiency bonus to the attack. They target fort, reflex, and will. Those are typically lower than AC.
There are backgrounds that allow this. Born Under a Bad Sign and Auspicious Birth allow you to replace CON with your highest ability score for starting hp (but not surges).
Really depends on the class. For my AD rogue, 20 doesn't hurt too bad as the entire class is based off dex. The 16 is still great for riders, and the other stats haven't come into play too much. Haven't tried pumping any other class to 20 yet.
An opportunity attack is not an immediate interrupt. It interrupts the action that provokes it, but an OA is not the same thing as an immediate interrupt. One is an immediate action, one is an opportunity action.
Linguist. I played a changeling assassin, and the shape change was useless 95% of the campaign due to language barriers. Unfortunately, 13int is expensive for an assassin.
Str paladins suffer more from feats like mighty challenge only working with DC and not DS... though the half-orc dragon article gives a feat that allows str to apply to both. The feat also stacks with mighty challenge!
Weapon damage isn't a huge factor for rogues. Daggers are only d4 and are basically the best weapons a rogue can wield because of the +1 to hit and the Daggermaster Paragon Path. Hitting is the most important thing, making sure you get sneak attack and applying all those nice rider effects on...
