    I have advocated this. Beyond that, 5e variants like Level Up provide much more choice within the class system, so two characters with the same class and level can be quite different.

    Well, in my games the fiction has priority over the rules (which IMO should adjust to fit it), and I like lots of prep anyways.

    I appreciate and would implement the simulationist reasons, but I don't think it would be completely worthless or that such issues couldn't possibly be worked around, so I say go for it! PCs can do what they want (to the limits of their own abilities) in my games anyway.

    I generally prefer to work from the other direction (adjust the mechanics to fit the fiction).

    Like I said, I'm working on the "gun to your head" issue. I worked through design on this with a buddy of mine for a while, but an unrelated falling out has left me trying to finish the work on my own.

    I've never played in (that I know of) or run a game in which improvisation on the GM side is the primary generator of play. It's always there in an RPG, of course, but for my games prep is more important.

    To be fair, if there's a diagetic reason the character doesn't fear crossbows then it absolutely is a sim agenda. For other uses, I think adjusting the rules for "gun to your head" situations can help. I've been working on stuff.

    That's a neat idea, and IMO probably a better way to organize play such that it can be discussed with less acrimony.
