Parties with two defenders and two leaders tend to have an easy time in encounters. Artilleries and Controllers should be your worst enemies. Have you fought much of them?

For a group of well built (not optimized) and well coordinated PCs I can totally see how they wouldn't sweat on encounters of their level + 1 and some level + 2 encounters. Coordination being the key factor here.

IME, Strikers are the best targets for NPCs, followed by Leaders, Controllers and finally, Defenders. Of course, it usually depends on the specifics of the combat.

I think that "a power misses" is defined as when "all targets are missed", so it shouldn't apply in the case of Twin Strike. Even then, the RAI is clearly to deal damage when your miss would lead to a "no damage dealt with this power" situation.
