    Sekhet-Aaru was the final gate. There were several possible destinations you could be sent to prior to that, starting with horrible and slowly getting nicer as you moved through each gate towards Osiris. And the gates were guarded by monsters if you tried to go past a gate you did not earn.

    Kelemvor was found guilty of incompetence due to humanity, because too much good in the prime material plane would unbalance the cosmos. So people go back in the wall. The way I understand it's a fairly peaceful boring process these days. None of that horrible torture business.

    AO wouldn’t do anything unless Vecna was poised to succeed at some Die Vecna, Die level shenanigans. Heck, AO didn’t even step in to stop Acererak who would have massively imbalanced the Realms.

    If Acererak can get to the Realms, so can Vecna, but probably only as an avatar or sneaky presence. But as others have noted, Avernus is not in Realmspace in the first place, so it’s not an issue.
