Just for fun: tell us about the most recent PC death in your current or most recent game. How did they die? Was it your PC? Were you the GM? Was it "deserved"? Was it up to the player? Was it up to the dice? Was it someone else's fault? Like that.
In my high level, almost done D&D 5.5E campaign, a few weeks ago the PCs faced down a great wyrm red dragon to bind it with a Orb of Dragonkind. During the battle, it managed to recharge its breath weapon more than expected and the poor bard just could not keep up with the damage output. She was killed -- but the PCs won and soon she was revived and back to singing goth-punk-metal ballads -- now with one about her own demise! It was not really anyone's fault -- the dragon just got a lucky with three rounds in a row of breath weapon.
