I chose 4, but I wouldn't call it "realism" or "historical".

And I have nothing against flying ships and mile-high towers either - those things might appear in my game - but they remain rare and fantastical - and in a combat you might lose a limb or get disemboweled (I use a crit chart) - and if you walk the streets of a city the commoners will be throwing the contents of their chamber pots out the window into the open sewer, and in some cultures women are treated like chattle, and suspicion and fear of the other is the most common philosophy, etc. . etc. . .
