Although death is cheap in CoC, it doesn't really fit my definition of grim'n'gritty. But that's probably a matter of play style :).

I'm not into grim and gritty, but Grim Tales is worth getting, anyway. It's a nice view on the wheels behind the d20 system. And I like Coc :).

Your welcome. I forgot to add another non-setting book that can be easily used to create a setting around- Goodman's Complete Guide to Doppelgangers. I find The Book of Unremitting Horror to be slightly disturbing (it is full of splatter), but the CGD is just plain scary. It adds new phases to the life cycle of the doppelganger and tells why they replace people. And it is a pdf.

Minions 3.5 from Bastion (only as a pdf) has a template for a creature similar to the one in the movie Phantoms. It is called the crowd and it has the feel of the CCD. There are several other monsters in it that can be used in horror games with the haunted house that rebuilds itself being my most favorite. But then again, it is not a setting obviously.
