I'm curious what changes have to be made when adapting a product from 5e to ToV. Are there some little mechanical differences like in skills, or are the beasties amped up to match ToV's monster math?
 

On the monster side it's not all that different, other than as you said the math being a bit different. There are some larger differences just in how encounters are set up and the format and terminology changes, and when we have adventures with supplemental rules that are on the player side (like new spells and character options) there are bigger differences there.

If you don't mind doing some work yourself on the fly or in prep, I think it's certainly possible to run one version of the adventure with the other system and have it work out okay.

Obviously going from something like 5e to PF2e is a much bigger hurdle, but much more obvious of what has to be done. I wasn't sure of what subtle differences would be made between 5e and something like ToV or A5E.
