D&D General Looking for a magic item with an effect similar to the word of recall spell

One more thing… if two potions could do the trick but you feel it’s too cumbersome to pull off the escape with two items, you can by the RAW mix potions, and the results are quite interesting. See Potion Miscibility in the DMG. There is a table you can roll on to determine the effects or side-effects of the combined potion. You would probably want to pick a specific outcome for that roll but anyhow. The full table of effects is well worth a read regardless, and I’ve been toying with the idea of a character concept that leans heavily into that to fish for some of the more interesting outcomes on that table 😅

Came across this today while flipping through my Rules Cyclopedia… in case your hag held on to an old magic item from a bygone era 😁

It requires being a magic-user though, so it wouldn’t work for your scenario… but I thought the mechanics were quirky and interesting 😅
 

EDIT: a Ring of Spell Storing is probably the easiest way…!

This was my first thought. It seems like the most obvious solution. Pick any transportation spell you want.

If you need to conceal it, make it an earring or a bracelet or something. If you need it to be a one-time-use item, have it be damaged so that it no longer allows spells to be cast into it.
 

Other options to consider are things that make one invisible. Fairly easy to get away when you can't be seen, but does leave a trail that could be tracked conceivably. Mislead, invisibility and improved invisibility items would be good here - cloaks, boots, potions.

There's also plane shift, etherealness and gaseous form that could be used. The latter is somewhat dependent on those seeing the attack not have magic to harm the user and them having some "escape hatch" (a wall with an ajar window or door, a manhole or some other small gap other can't follow). These could come as potions, talismans, amulets or possibly be in a rod (or staff or wand). Maybe even a feather token?

Heck, dropping darkness or fog cloud can make great cover for a getaway as well. On teleportation, thunderstep could be extremely useful for a getaway + distraction/cover.

Might also consider a ring of spell storing. Don't need to be able to cast spells to use it as long as the stored spell is 5th level or lower.
