You might be thinking of Lost Destinations, which is based on the freakish and horrific stories of the real world. The first volume covers one town in which one can find: An abandoned neighborhood which, rumor has it, had every last inhabitant slaughtered by devil worshippers. A long, winding road to nowhere, haunted by albino pygmies, huge white dogs that can chase cars at high speeds for miles, a killer curve that has thrown many cars off the road and drowned numerous people in the lake below, a ghost that haunts a small bridge by throwing things back at anyone dropping stuff over the side, a collapsed castle often used as a place of worship by satanists... Oh, and did I mention a mafia hit man turned serial killer used the woods off the road as his private dumping grounds for his victims? The Hookerman Ghost, victim of a railway accident who know murders along the rails in the town. The Trail Stalker, a ghost on a chunk of the appilachian trail that will either help you or kill you, depending upon what time of day you meet it... Strange fungi that fire sticky gobs of spores at anything that moves... A safari zoo abandoned in the late 70's, where they say they let many animals loose to run rampant in the hills (its a known fact this is true of the peacocks now native to the area) in order to cover the embezzling manager's escape, and they left many animals trapped in the park to die of starvation. Since then a number of disturbing things have happened on the old park property... I could keep going, but I think that should be enough to let you know if it is the book you were thinking of. It was due out last May, but there have been numerous delays since then, not the least of which is the absence of an OGL Horror logo to put on the book, forcing the entire thing to be revised to D20 Modern only. Looks like the revised projected release date is October 1.
