Polearm Gambit, Combat Superiority and Antipathy gloves are an excellent and viable combination. However it doesn't help any against certain things. If you want to give your players a challenge, throw an encounter of just enemy artillery at them and give the monsters a well-defended position. An Adult Blue Dragon roosting in a canyon could provide an exciting encounter, as the players need to dash from positions of cover to reach a less steep ridge that they can climb up. A few Githanki warriors hold the line (and immobilize PCs with telekinetic grasp) while a pair of Redspawn Firebelchers attack from a distance. What are the other defenses of the Paladin? If he or she has a poor will defense, substitute a Githyanki Mindslicer into the above mentioned Githyanki encounter. The Redspawn Firebelcher can target his Fortitude and the Mindslicer his Will. In reference to your specific questions: 1: I don't think that'd count as difficult terrain, despite having the same mechanical effect. 2: Charging into someone's reach does count as provoking an OA. The enemy can use a move action to enter his Polearm Gambit, take the hit, turn their standard action into another move action and move adjacent though. 3: Since you're making a basic attack as part of a charge, it should take into account the weapon's reach. You'll stop in the square in front of the character, he'll still get the OA, but you'll then make your attack (presuming you live). I hope all of this helps!
