I think "The PCs blew it up" is the most likely explanation, judging by my experience.

Other options:

Going with the Doctor Who option theme, it is still there but under a perception filter. Now that you've seen it before and are thinking about it you can force yourself to see it now. It only took a generation change for it to be mostly forgotten and not referenced, though old records might exist that people actively ignore and not think about. Is this an undermountain megadungeon that nobody on the surface has explored before or does one hermit in a hut on the side of the mountain really want their privacy?

Meteor swarm? That's nothing, I heard about this wizard who researched a summon mountain attack spell!

Giant mimic mountain? It migrated a couple generations ago. If it ever really existed, I think it is just a neat folklore story.

The city's patron god lost it in a bet.

Did you know invisibility spells originally were permanent and could be cast on objects?
