I have never heard fudging referred to the "Gygaxian Approach" before and I have to wonder where that comes from.
Especially considering that he frequently preached the exact opposite. I don't know that Gygax had a consistent expressed approach.

Combat - B

Trinkets/Items or Situation or NPC Name/Trait - B unless its repetition, boring or I cannot immediately make it work then I reroll or pick something more appropriate from the list.

I know that Gygax said to throw out die rolls that did not make sense in the context of random encounters, treasure or whatever. I do not recall him ever giving the advice of fudging in combat to make the game more fun for the players.
