What are your house rules?

okay - I lurk here all the time. I don't usually hit the house rules forum so i don't really know the answer to my question. I am going to be starting a new campaign soon and was just wondering what house rules are being "generally" accepted as better, improved, unbroken, etc.

I would rather not see harm, heal, poly, etc. I realize these are broken and are usually changed by the DM, what I would like to know is - how are they changed? Do they get nerfed all together? Saves allowed? What???

You know, it's no big deal, and I have a nice page with my own house rules, but I guess I just don't really have the time...
