Where is marking described?

I can't seem to find a rules description for basic marking.

Under Fighter combat challenge and paladin divine challenge they say it marks a target and describes what happens to the marked target (if attack others -2 to attack and provoke opp attack or -2 to attack others and take radiant damage). Other powers such as level 1 cleric encounter power healing strike just says if it hits the target is marked until the end of the character's next turn without saying what the mark does. A bunch of monsters mark as well (usually soldier ones) without describing any effect for the mark.

Marking is not in the PH index, and I can't seem to find a description for basic marks.

Where does it say explicitly what a generic mark does? I'm guessing it is -2 to attack others as that is the common factor in the two described marks but something more explicit as a rule reference would be nice to be sure.

It's in MM2 (at least according to the compendium)

When a creature marks a target, that target takes a –2 penalty to attack rolls for any attack that doesn’t include the creature as a target. A creature can be subject to only one mark at a time, and a new mark supersedes an old one.

Defenders get special bonuses on top of this penalty to further punish those who don't attack them

The Compendium will refer you to the most recent authoritative source ont he subject, under the assumption that more recent books have errata/rules updates incorporated and have clearer explanations of the rules. If it refered you to earlier books, you might not get the clearest picture of the condition, or get a version lacking essential updates.
