Please verify my monster damage chart

Hello all. I put together a new DM crib sheet that contains the new DCs, defenses, attacks, and other standard math expressions onto a single 5x8 printable page. I wanted to get you hardcore guys to verify that my monster damage math is correct. I based it off of Jhaelin's awesome work in the following thread:

Hey this is exactly what I need! Hopefully the damage expression experts, etc can comment on it in short order.

This isn't a math suggestion, but I'd suggest alternating grey and white lines. I've always like this in the D&D books. Its easier to follow the line across.

Do you by any chance have the actual spreadsheet available? I love this all-in-one-place resource, but I'd like to tweak a few numbers based on some houserules of mine.
