This is my attempt to give some lasting effects to environmental skill challenges by reducing the number of healing surges characters will get. I don't expect it to be a terrible burden, I'd be surprised if even the wimpiest character ever had 3 fatigue points at any given time.
Fatigue points represent the buildup of physical exhaustion over the course of days or weeks of strenuous or stressful activity without adequate rest.
After taking an extended rest, you lose one healing surge per fatigue point. If you have more fatigue points than healing surges, you get no healing surges that day but otherwise suffer no additional penalties.
Gaining fatigue points: Fatigue points accrue most often as the result of failing endurance checks during long-term skill challenges. For example, attempting to travel through a blizzard, withstand the heat of a desert, or spending days being rocked around on a ship in stormy waters.
Losing fatigue points: When you take an extended rest in a particularly amenable environment, such as an inn or, perhaps for a warforged, a workshop, you can attempt an endurance check (DC 10+half your level). Success indicates that you may discard a fatigue point.
