"As your party approaches the steps of the alter, you see several portals open up top and a slew of cultists gather around a tall and well-armored orc. As she turns to the mass, the orc calls out.
The elements themselves will rise up against the civilized world! Only the faithful will be spared!
You top the stairs and grab the attention of the cultists. Their presence appears to be creating a protective bubble around the orc as she preaches to them.
Immortality can be yours. But only if you pledge yourself fully to Yogg-Saron!
Here on the very borders of his domain. You will experience powers you would never have imagined!
You have traveled long and risked much to be here. Your devotion shall be rewarded.
The faithful shall be exalted! But there is more work to be done. We will press on until all of Azeroth lies beneath his shadow!

Have the cultists (x8 level 18 minions, 2 groups) protect the orc with their lives. When the party has defeated both groups, the orc calls forth more minions who sit by and become protected by her bubble even as she becomes vulnerable to attack. Read the following.
These are sacred halls! Your intrusion will be met with death.
During the combat, the female orc (level 19 solo brute) will attempt to cleave party members standing in close blast 3 of her. She also will whirlwind, close burst 2, if flanked. When the orc becomes bloodied, she becomes immune to all attacks until the end of her next round and flies as a free action 5 squares into the air. Read the following.
Who among you is devoted? You there! Step forward!
One cultist becomes vulnerable at this time and begins to run to the spot under the orc. The party needs to kill the cultist (3 successful hits against any defense) before it reaches the space at the end of the orcs next turn or it dies and the orc gets a +3 bonus to attacks and deals [2W] extra damage with attacks until the end of her next turn. The cultist is immune to movement changing effects during this time.

Do not expect your sacrilege... to go unpunished.
*Special* if the party chooses not to kill the cultist when the orc becomes bloodied, grant them one additional item of level 18 or lower as a reward.
