My ACKS campaign has had 12 PC deaths in the 16 sessions we've played since mid-June, in addition to 17 henchman deaths. It's gotten to the point where we have a page on our Obsidian Portal to keep track of the casualties. Here's the list as it stands currently:
Slain PCs:
* Yolanda, Joe's Bladesinger of Iana - Slain by giant spider crab venom in the dwarven fortress of Sandygates on the first expedition.
* Agord, Joe's Priest of Ammonar - Slain by a gelatinous cube in a pit in the dwarven fortress of Sandygates on the second expedition.
* Scarth, Drew's Burglar - Knocked unconscious by a gang of morlocks in their lair in the dwarven fortress of Sandygates on the sixth expedition; subsequently burned to death by Errgumun.
* Galivan, Tim's Protector of Iana - Knocked unconscious by a gang of morlocks in their lair in the dwarven fortress of Sandygates on the sixth expedition; subsequently burned to death by Errgumun, then resurrected as a women by Corinth, then retired.
* Errgumun, Tom's Elven Theurge-Torturer - Slain by a morlock in the secret stairwell in the dwarven fortress of Sandygates on the sixth expedition while attempting to escape.
* Carcophan, Tom's Theurgist - Slain by spears thrown by wights while levitating on the ceiling of the reliquary of the Bleak Academy during the first expedition. Subsequently returned to an accursed unlife by Ancaglon the Black.
* Urist Boatmurdered, Andrew's Dwarven Warden - Slain by self-immolation in the face of 17 angry berserkers in the Bleak Academy during the first expedition.
* Urist Handlepainted, Andrew's Dwarven Punisher - Slain by self-immolation in the face of 20 angry zombies in the Bleak Academy during the fifth expedition.
* The Albanian, Andrew's Explorer - Slain by a stream of poison gas by a dragon in the Crocodile Temple.
* Baron Garwyn I of Opportunity, Drew's Rhapsodist - Obliterated by Reapinggale at the end of Year 2, Month 2. Resurrection was not possible.
* Leo McCrane, Alex's Theurgist - Suffered a lethal allergic reaction to the venom of a man-sized killer bee during the third expedition to Fort Camarone. Subsequently resurrected, but haunted by dark forces.
* Corinth Ironsmith, Tim's Myrmidon - Slain by a cloud of yellow mold spores near the exit from the Goblin Heap during the third expedition to Fort Camarone. Subsequently resurrected, now mutters incessantly.
Slain henchmen:
* Lasai, Drew's Camp Follower - Slain by Scarth the Robber in the dwarven fortress of Sandygates to save on XP and loot sharing on the third expedition; subsequently returned as a ghost to haunt him during the sixth expedition, put to rest and given a proper burial by Durgrim the Sentry.
* Slick, Drew's Thief-to-be - Slain by giant insects while lost in the swamps between Deal and Opportunity.
* Kurtak, Drew's Viking-in-Training - Torn to pieces by skeletons during the first expedition to the Bleak Academy during week seven.
* Jimmy the Rat, Drew's Explorer-in-Training - Had a bunch of teeth knocked out by skeletons during the first expedition to the Bleak Academy during week seven; subsequently died of complications.
* Anroth the Clumsy, Drew's Acolyte - Beaten to death by zombies during the teleport debacle, where his -2 initiative from Dex literally killed him during the first expedition to the Bleak Academy.
* Turix, Tom's Man-at-Arms - Torn apart by hungry ghouls during the first expedition to the Bleak Academy.
* Derra, Drew's Blade-Singer - Eaten by ghouls during the sixth expedition to the Bleak Academy.
* Sorlof, Drew's Warrior - Eaten by ghouls during the sixth expedition to the Bleak Academy.
* Morik, Thomas's Assassin-Priest - Eaten by ghouls during the sixth expedition to the Bleak Academy.
* Calth, Tom's Seer - Force-choked to death by his duplicate from the Mirror of Opposition in the Bleak Vault during the seventh expedition to the Academy.
* Addamar, Andrew's Theurgist - Ripped apart by a wyvern north of Opportunity while pursuing the Crow's Syndicate at the end of Month Six.
* Jarol, Drew's Curate of Thoth - Spine severed by a wyvern's bite north of Opportunity while pursuing the Crow's Syndicate at the end of Month Six. Subsequently resurrected, now feared by horses and other normal domestic animals.
* Hulst, Tim's Marauder - Slain by wyvern venom north of Opportunity while pursuing the Crow's Syndicate at the end of Month Six. Subsequently resurrected, now feared by horses and must perform a Quest for his deity (retrieval of the Spear of Aldus for his revered ancestor, Urvin the Wrestler of Crocodiles).
* Muldred, Drew's Man-at-Arms - Torn apart by a wyvern north of Opportunity while pursuing the Crow's Syndicate at the end of Month Six.
* Myu, Tom's Elven Arcanist-Avenger - Slain by poison breath of Great Crocodile during the expedition to the Crocodile Temple
* O-Kayu, Jason's Elven Warrior-Seer - Slain by an invisible stalker while sleeping during Year 1, Month 12 after a spell to fix his broken neck angered unspeakable powers.
* Bhoskar, Drew's Dwarven Shieldbearer - Devoured by an ankheg during Month 4 of Year 2. Subsequently raised from the dead using only his beard, recovered from the ankheg's stomach. Now feared by horses and normal domesticated animals.
One of the worst (er, best?) incidents was the Morlock Massacre, where all the PCs were killed but all of their henchmen survived.
