Cloak of clown form

The cloak of clown form makes the wearer look like a clown!
For example a 1/2 orc would probably get little balls/bells on his/her tusks and gain blindsight as a added feature, the better to act silly for different races/children.
What spells would be needed for this item?

Wouldn't a hat of disguise already make you look like a clown, if that's what you envisioned when you put it on? (And I'm not seeing the correlation between "clown" and "blindsight.")

Blindsight is for blindfolded pie tricks mostly

...anyone else think this should really be a prosthetic nose or oversized shoes/pants, not a cloak?
