Custom Racial Substitution Levels?

I really love the concept of racial substitution levels from Races of Stone, and I'm curious if anyone here has created any of their own.

I haven't seen Races of Destiny yet; does this book have substitution levels as well, and if so, for what race/class combinations?

RoD has substitution levels for:

Half-elf bard
Half-elf fighter
Half-elf ranger
Half-orc barbarian
Half-orc druid
Half-orc paladin

Planar handbook has substitution levels for all core base classes: Barbarian, Bard, Cleric, Druid, Fighter, Monk, Paladin, Ranger, Rogue, Sorcerer, and Wizard.

They're not exactly racial substitution levels, but must fill at least one of those prerequisites to get a planar sublev:

• A full rank in Knowledge: The Planes
• At least one Heritage feat
• Have the extraplanar subtype when on the material plane
• Have visited a plane other than the material plane.
