That's a very hard question to answer without knowing the nature of the custom gear involved. If the custom gear is not superior to what other party members already possess, thee seems little harm in allowing it into your game. I've done it with some success in the past.

I have no problem with replacing wooden hafts with metal ones. This was actually quite commonly done. As a modern example, we have baseball bats of both wood and metal construction and I've never heard a baseball player complain about the "balance' of an aluminum bat.

Ditto for the request for an Age of worms one. These are fantastic! Great job.

The Amalgam Template from Green Ronin's Advanced Bestiary also allows you to "mash" any two creatures as well. It's worth a look.
