You must also take into account the fighter's "blind-spot". No matter how many hits he'll take, the fighter won't run away from mere orcs. Especially if there are chicks watching. "Run? HA! I have 12HP left, no problem...."

Improved critical is cool, or if I improve monsters and have no specific place to put the new feats, I always use the saving throw enhancers: Iron Will, Lightning Reflexes, and Great fortitude.
