DMG definitely, followed by my PHB. My MM is still in excellent shape for some reason despite my using it a fair bit. It's like it's blessed or something. :p

Our weekly sessions are about 3-4 hours long. PCs still level up about one every month, month and a half, I just give out a little more story based XP and try to make the fewer combats a little more intense.

In my group we just eliminated the possibility of rolling a 1. Fighters roll d6+4, Barbarians d6+6, Rogues, d4+2, etc... For spellcasters we just let them have the 4 points. That's enough of a penalty in my book. :p

For any adventurer with a flair for the dramatic "Have at thee foul denizens of the dark dimensions!" I got a couple strange looks for that one. Can't imagine why. Seems perfect for a rogue. :D

Now this is an interesting idea. Character creation in Traveller really helped establish who the PC was and a similar concept could go a long way towards bringing in new PCs to an existing campaign by giving them an established history in it. Now if only I had the Traveller books to try and...

One of my players mentioned a method he uses. Using the rules for wealth by character level in the DMG, he lets his players buy what ever they want for gear so long as no one item is worth more than half of his gp total. For 9th level this means no item worth more than 18000gp. Powerful items...

Actualy you could use the Elven angle. You've already stated that the people in this area don't really remember what happened 500 years ago. If you limit human contact with Elves somewhat, they could become a source of information about what is going on that the PCs would have to travel to...
