Attacking with Tiny creatures

Tiny creatures have no reach (0) so to attack do they have to be in the same 5 foot square as the creature they want to attack?

Is there a + or - to the larger creature?

How do they affect flanking?

Yes. Creatures with O ft reach suffer an AoO when entering a creature's space.
Is there a + or - to the larger creature?
If you mean AC or Attack bonuses, yes, they are already figured in.
How do they affect flanking?
Don't know. Good question.

Since they can "make melee attacks" in the square they are in, they threaten the square they are in by definition.

Conceivably, they could flank an opponent, if the DM decides to keep track of where everything is inside a single square.
