As far as I can tell, the basic math is here, but the mechanics and various other features need to be altered. Read the Monster Manual and try to arrange its abilities like that. I can try my hand at this, but I don't know much about Lovecraftian Monsters. I have tremendous respect for him and his work and have a copy of his completed works, but have yet to sit down and read it. First and foremost, the features you describe aren't conditions. You can find a list of the various conditions in the back of the Player's Handbook for 5th Edition. Next, Frightful Presence isn't a reaction. It's an ability listed under a creature's Actions In Combat. It's Multiattack Ability lists how its combat options can be used in a fight. Thirdly, you aren't using the proper set up for the math of the creature's attacks and damage statistics. When determining a creature's bonuses to weapon attack rolls and the damage of those attacks, you take into account its proficiency bonus (Based upon its CR) and add that to its relevant ability score modifier, and then calculate damage for the attack based upon the average die rolls for the dice used and adding its damage modifiers. Fourthly, if a creature has immunities or resistances to damage types, they are listed separately from its condition immunities and unless a creature has immunity to all aspects of a condition, it does not have condition immunities, such special vulnerabilities must be specified under the creature's Special Qualities section. Lastly, there is no longer determinations of different extents of flight maneuverability.
