I have had players loose all their hit points and instead of dying, they entered a catatonic state of paralysis that the healer had to get to work on to help the player recover because the attacks were primarily psychic in nature. I have also done the same thing with people loosing all their hit points due to poison attacks.</p><p></p><p>Also, I think you are over-simplifying the role of healing surges in 4E. There are plenty of situations and magical effects where both HP and healing surges do not replenish back to full status overnight.
