    In Manual of the Planes, page 107, powers of pandemonium, it says that "Vecna mantains a hidden tower in the heart of Pandemonium". Vecna is known, among other titles, as the "Lord of the Rotted Tower" (Open grave, 212), maybe this title hints to this secret tower in Pandemonium.
