You don't need spiked chain proficiency, a Guisarme and armor spikes (plus unarmed tripping) will cover the same threatened area as a spiked chain and saves you a feat. The forced feats from the animal form ARE a downer, of course, really eats into your allotment. But a Human Fighter 1 can have the other 3 right at level 1, then you go into the lycanthrope template. Is it possible to use a monster level progression like was posted above? Even something as simple as starting Fighter 1 / Brown Bear 4 / Level adjustment 2 and just take bear HD as your next two "levels"? If you can't get werebear, none of the others are worth it for you. Best bet after that is probably a Wizard, believe it or not. Get the highest Con and Int you can afford, possibly even valuing con over int since you need it to overcome the d4 HD (do try for Int 18 after magic items and level 4 increase, though). Strength doesn't actually matter, dex you only need enough to not suck at initiative. Be a Transmuter. Use the Polymorph spell, and pick the biggest, strongest form you can meet the CL and HD requirements for. Post level 7, pick up a level of Fighter and enter the Eldritch Knight prestige class. Ideally you'd be in it already, but you need 7th level caster to have polymorph, and you definitely want to start the game with it at all possible. This means 3/day you can become quite the tripping machine, since str checks don't care about BAB, and touch attacks are easy.
