D&D 2E Does Anyone Have the World Builder's Guidebook?

I bought a used copy of the World Builder's Guidebook a while back, but it was missing the blank forms that come with it. Does anyone have any of the blank forms? If so, would you mind scanning them and sharing. I'd like to recreate them in Adobe InDesign (and then share them with the community in PDF format) so that they can be printed up by whoever needs them.

Yup, pdf is your friend here.

Word of warning though, some of the map blanks don't really match up with the book's text. It's not a really big deal though. The blanks are pretty adequate for mapping. One of the blanks is a square grid, but you don't need to worry about that; you got graph paper, right?
