Some things it could do with:
Monsters separately colour-coded from PCs. Ideally, a choice of several colours to assign, with a default kicking in if you don't make a choice.
More consistency between the controls for adding PCs and adding monsters. Yes, you need different options, but the main buttons should stay in the same place.
A round counter. Often, a DM will have new events trigger after a certain number of rounds, and adding a counter that increments every time you return to the top of the initiative list would be a simple and useful addition.
Some ability to quickly account for Readied and Held actions, or any change at all in initiative order. I couldn't find any way to do this, and it's an absolutely essential feature in an initiative tracker.
