From D&D to Fiction

A few things I've read lately have got me thinking. How many fictional worlds began as D&D worlds? I've read in different places that Feist's Midkemia and Blizzard's Azeroth began as game worlds and grew from there. Anyone know of any others?

Elizabeth Moon's 'Deed of Paksenarrion' started its life as a D&D game, so that world came from gaming. (Hey, it even has a thinly disguised Hommlet!)

Well, Dennis L. McKiernan's world of Mithgar came (in part) from a long-running Rolemaster campaign of the same name. In manga, I understand that Record of Lodoss War and Bastard! both originally arose as the result of AD&D campaigns.

Lawrence Watt-Evans 'Ethshar' setting started that way, or was influenced by his campaign.

The 'Wild Cards' series of books was inspired by George R R Martin's Superworld campaign.

Brust's 'Jhereg' series obviously owes it's roots to a D&D game.
