How do you handle PC plot twists?

You need to have a out of game discussion with your players. If you introduce character specific plot twists/secrets, the Players need to be ok with the possibility that their Characters may have to act in ways that other Characters in the party suffer negative outcomes from.
Example - Player Bob is running Paladin Charlie who belongs to an order that is oath bound to kill all werewolves on sight.
Player Fred is running Wizard Sparks who has a secret that he is a werewolf.
Player Sally is running Cleric Neutral who follows a conflict solving deity.
Can Bob, Fred and Sally handle the results that happen after Charlie learns that Sparks is a werewolf? Possible results could be:
Charlie follows his oath and kills Sparks.
Charlie doesn't follow oath and losses his Paladin status.
Sally manages to cure Sparks of his lycanthropy and avoids either of the first two results. This result assumes that Sparks wants to be cured.

If the players are cool with this, then have fun. Such intraparty conflicts can be some of the most fun memories of a campaign. If the players can't deal with it, then such conflicts can fracture a Player group.
