D&D (2024) Is there any confusion between Poison Damage and the Poisoned Condition?

I like Weakened as a condition a la stunned or prone! Poisoned could be a part of that condition, indeed, while what is now Poison Damage could be Toxic Damage and Poison could be key-worded to refer specifically to the concoctions that cause various conditions or detrimental effects.

I also like the idea of an optional rule that upon becoming Bloodied (below 50% of your HP), you would also be Weakened, if you aren't already.
