After reading the complaint about how "FAST" creatures ought to be able to act more often than slower creatures, I through "why not just use the system that our group uses"?
Initiative for multiple attacks:
If a character has multiple attacks in a round (whether through high Base Attack, 2-Weapon Fighting, Haste, or combined), divide the character's initiative roll by the number of attacks (always rounding down), and subtract that result from the original initiative: the character’s second action occurs on that second initiative count.
E.g.
The PC has two attacks and rolls a 19 for initiative. Divided by 2 (and rounding down) would be a 9; therefore the PC will act on Init 19 and Init 10.
A PC with three attacks rolls a 23 for Initiative, divided by 3 is 7; the PC acts on Init 23, Init 16 and Init 9.
So, at the beginning of the round (or combat), everyone rolls initiative. Starting with the highest initiative roll, those characters go first; count down and when you get to an initiative number that corresponds to someone else's initiative, then that PC/NPC/monster goes next. One of these higher-level PCs with multiple attacks (if they choose the full/iterative attack option that round) would take one of their allotted attack rolls on each of the resulting initiative counts.
