Sneak Attack (am I reading too much into this?)

Regarding sneak attacks and denied dexterity bonuses...

I'm jumping on this thread to ask a question related to this same subject. Could anyone out there list the circumstances when you are denied a dex bonus and a rogue has the opportunity to do sneak attaclk damage? For instance, I know that a rogue gets to do sneak attack damage when:

1) The opponent is flatfooted
2) An invisible rogue attacks an opponent who can't see him/her
3) The opponent is paralyzed

BUT, does the sneak attack damage aply when:

1) The opponent is in a grapple
2) The opponent is pinned
3) The opponent is prone

???

Sneak attack damage applies to an opponent in a grapple, unless the rogue is part of the grapple: a grappling character loses his Dex bonus against all opponents except those involved in the grapple.

A prone opponent does not lose his Dex bonus, and thus isn't automatically eligible for sneak attack.
