I use the same method I use to resolve any other tie in initiative - highest initiative modifier goes first, or if the modifiers are the same, flip a coin.

Generally, if there's a conflict in timing, the creature whose turn it is decides on the order. Hypersmurf's suggestion—or an initiative check—also works.

Things happen sequentially in D&D, so readied actions are taken one at a time.

Now, the first PC takes his readied action, and kills the foe.

p291 .. if the trigger doesn't occur or you choose to ignore it, you can't use your readied action, and you take your next turn as normal.

Well, the trigger is there, and you may take your readied action .. feel free to ignore the trigger.
