I figure the easiest ruling is poisoned condition, but smoke eventually kills.

Anybody have smoke inhalation rules from old editions? Mine are buried in storage.

Often smoke incapacitates so quickly that people are overcome and can’t make it to an otherwise accessible exit

Still, two rounds is too fast to start rolling damage. An average human has 3 hp and isn't going to fall unconcious after 12 seconds in heavy smoke. This is from an article on the toxicology of smoke inhalation from a firefighter's website:

Early signs of toxic exposure include a decrease in cognitive ability, hypotension, confusion, anxiety, a decreased level of consciousness, a rapid pulse, poor tissue perfusion, rapid breathing, and dyspnea.

Dyspnea is shortness of breath.

So all of that stuff precedes passing out from lack of oxygen.

Edit: found this quote ...

Fire officers told me on three occasions that her decision was critical as smoke inhalation kills in two minutes.

So damage starts after 2 minutes in the smoke.
