I have just uploaded Form Fillable Spellbook/Spell Cards to the downloads area.
This PDF contains no spell descriptions, it is a blank template that the user must fill out.
I made this template so I could print out mini spellbooks for my players to use so they don't have to fight over the Player's Handbook as much, and to have only the spells that are relevant to their chosen classes. My idea was to print out the form (after filling in all the relevant info) on both sides of a page, and staple the pages together down the middle. Folded in half, it makes a nice little spellbook. These can also be cut apart and used as cards if you prefer. Comments and critiques welcome!
